Managing Medication for Cardiac Rehab Patients | Vibra Rehabilitation Hospital of Rancho Mirage

important video message. My name is Tien. I’m the Director of Pharmacy at Vibra Rehabilitation
Hospital of Rancho Mirage. February is Heart Month here at Vibra, so
this month’s social media video is going to focus on heart disease and general cardiac
health. Although we can treat and manage a myriad
of myopathies and neuropathies, along with their associated co-morbidities, I wanted
to give an example of cardiac heart failure myopathy, and how we can help you manage your
care. At Vibra Rehabilitation Hospital of Rancho
Mirage, our attending physical medicine and rehabilitation physicians work with internal
medicine and also cardiology consults to manage clinical indications, such as CHF, or if you
have had a post-heart attack, or CABG. My role as the pharmacy director, is to oversee
our clinical pharmacists to actively manage your Coumadin, or any other anticoagulants
you may be taking to get you to your INR goals before you go home or back to the skilled
nursing facility. Our clinical team, including the physicians,
are here to care for you 24/7. We will be more astute to changes in your
health, and act immediately for your best outcomes. So how do you know if you qualify for our
level-of-care? Our team will talk with your doctors, whether
you’re staying at the acute care hospital, or skilled nursing facility, or even your
PCP. They will evaluate you and see if you need
